Co-Pay Assist Plan

Note: Stated below is only product information. Always seek a qualified advisor for advice when in doubt. Health insurance policies not suited for you will only affect your ability to finance you’re future healthcare needs.

This plan is designed only for public service employees under the Comprehensive Co-payment Scheme (CCS). Of the hospital bills and certain outpatient bills for the treatment of major illnesses incurred by you and your dependent, the policy will pay half of the co-payment.

Under the Comprehensive Co-Payment Scheme, you will have to co-pay 15% of your own bills and 40% of your spouse and children’s bills. This policy will pay 7.5% of your hospital bill and 20% for your dependents.

Coverage

  • In-patient hospital treatments
  • Outpatient treatments for the following:

      • Kidney dialysis
      • Chemotherapy
      • Radiotherapy
      • Cyclosporine
      • Erythropoietin

Note: The plan covers only the medical benefits provided by the employer. It does not cover optional benefits and costs due to upgrading of wards.

Premium
*GST is inclusive in premium rates and subjected to revision. A minimum premium rate of $10 is payable per proposal.

Note: First year premium are to be paid by cash or cheque. Renewal premium will be paid by salary deduction once a year.

Example
Of your total $4000 hospital bill, you are required to make 15% co-payment of $600. With this policy, we are to pay half of the co-payment, i.e. $300.

Of your dependent’s total $4000 hospital bill, 40% co-payment is $1600 and the policy will pay half of the co-payment, i.e. $800.

Please submit your Proposal form and Authorization for Yearly Deduction form to NTUC Income through your HR/Personnel Department to apply.
